Student Physical Exam Forms and Immunizations Records
In order to comply with Illinois School Code, Section 27-8, it is mandatory that all freshman students, students new to high school and transfer students, who were not previously examined for ninth grade, have a PHYSICAL EXAMINATION along with verification of the minimum immunizations on file before starting school.  Please read Class of 2022 Physical and Immunizations Requirement Letter.

Physical exams must be completed on the Certificate of Child Health Examination Form.

Student Insurance
Maine Township HSD 207 is not liable for accidents which occur at school.  It is important to understand the school/district is not responsible for medical payments for your child.  If your child is injured during ANY SCHOOL OR ATHLETIC ACTIVITIES, all medical charges are YOUR RESPONSIBILITY.  For the 2018-19 school year, the school district has a student/athletic accident insurance plan available for parents/guardians to purchase.  Information can be found here.

Health Services Office - Student Physical Exam Forms and Immunizations Records

The Maine South Health Services office will be an important resource for you and your child. The school nurses are located in the Health office and provide services to students who become ill during the school day. We are also responsible for maintaining student physical exam forms and immunizations records.

I have linked some important information regarding the health and immunization requirements for your incoming 9th grade student. All incoming 9th grade students must have a current physical examination and up to date immunizations on file in Health Services in order to attend Maine South High School. Maine South High School~ District 207 has a policy of first day exclusion. This means that students who do not have a current physical exam and immunizations records on file will be excluded from school until such requirement is met. Please return the completed and signed health forms which includes the health history before July 15th or as soon as you can before the first day of school. It is important to note the State of Illinois Certificate of Child Examination form can be used as a sports physical but the IHSA Pre-participation Examination form will not be accepted as a freshman physical. If your student is planning to participate in sports, please make a copy of the physical and send to the Athletic Department for their file.

Please read Class of 2022 Physical and Immunizations Requirement Letter.  I am also sharing the  School Based Health Center Welcome Letter along with Scope of Services our clinic has to offer.

We look forward to having you son or daughter here at Maine South.  If you have any questions, please visit our Health Office Page or do not hesitate to call or email our Nurse, Brenda Keeley at 847-692-8199 or

Activities Fair for Class of 2022 - March 14, 2018

We would like to invite you the Maine South Activities, Athletics, and Fine Arts Fair that will take place on Wednesday, March 14th.

The evening will begin with a brief meeting in the auditorium with the building administration and current students.  From there the Class of 2022 will be escorted by our Junior and Senior Hawk Pride mentors to the field house and spectator gym.  There they will find table after table of opportunities to get involved at Maine South.  We will have all our clubs, athletic teams and fine art endeavors present to answer any questions.

While the students are checking out the fair, the parents will have an opportunity to meet with our principal, Dr. Ben Collins, to help begin your journey of having a high schooler at Maine South.

The evening will conclude with the parents meeting up with their students at the activities fair.

Times will vary depending on what current Junior High your student attends.

Students from Emerson, St. Paul of the Cross & Pennoyer will start at 6:30 p.m. in the Maine South Auditorium.

Students from Lincoln, Mary Seat of Wisdom and all other schools will start at 7:30 p.m. in the Maine South Auditorium.

Parents, this event is an opportunity for you to bring a copy of your student's birth certificate.  You can bring a copy of the birth certificate, or bring in the original and we will make a copy for you.  We will have copiers and drop off boxes available in the Back Gym.

Don't worry about making dinner on the 14th because you are invited to join in on a Maine South traditional fundraiser - PIZZA MADNESS!!  For only $5 a person you can try up to 10 different pizzas from local pizzerias.

This event takes place in the student cafe from 5:30 p.m. - 7:30 p.m.
